Position:  RN Clinical Supervisor – Home Care

POSITION SUMMARY:  The Registered Nurse Clinical Supervisor (CS) is responsible for coordination and management of patient care. The CS is responsible for the supervision of clinical personnel to ensure that care and services are delivered appropriately.

What does a Registered Nurse CS do?

  • Supervises clinical staff and ensures the quality of patient care under federal, state and local regulations and accreditation standards
  • Monitors clinical documentation for completeness and accuracy and assures that it meets standards for good clinical documentation and successful billing
  • Oversees the completion and implementation of Plan of Care
  • Reviews and approves orders written by field staff (LVNs and RNs) to ensure completeness, correct format and accuracy
  • Ensures effective coordination of care with physicians, referral sources, interdisciplinary team, and other individuals as necessary
  • Secures orders for evaluations as needed
  • Reviews Face to Face (F2F) encounter documentation and follows AccentCare F2F escalation process. Facilitates F2F encounter appointment as needed
  • Assures case conferences are conducted and guides the discharge/recertification process. Documents decision on a case conference coordination note within 48 hours (Not Applicable to Float position)
  • Coordinates and facilitates interdisciplinary communication including physician communication of client status changes, lab and treatment reports, and changes or updates to the Plan of Care
  • Reviews accuracy and completeness of the following: wound score deviations, client care type assignments, and client unusual occurrence reports as well as entry of unusual occurrence reports into the STARS reporting portal
  • Reviews SHP OASIS audit alerts and confers with field clinicians to ensure accurate OASIS item assessment as needed
  • Conducts routine record reviews and communicates findings and recommendations to Director of Patient Care Services
  • Reviews records that have triggered a potentially avoidable event alert
  • Performs back office OASIS as needed including TIF, Discharge, and Death at Home
  • Resolves any outstanding issues for the final claims billing process as needed
  • Serves as a mentor and clinical resource for staff
  • Is available at all times during operating hours to assist clinicians as needed
  • Performs clinical nursing and supervisory visits as necessary
  • Evaluates employees’ performance and makes recommendations regarding continued employment, discipline and pay increase considerations Keeps the Director of Patient Care Services informed of any serious patient care and personnel issues
  • Responsible for effective utilization of staff including assignment, reassignment of duties, calling additional employees where appropriate
  • Hires, monitors, and supervises clinical personnel
  • Takes report from field staff and intervenes appropriately regarding new admissions, changes in condition, and new orders
  • Initiates and executes disciplinary actions (formal and informal) with employees, including warnings (verbal and written) suspensions and effective recommendation of additional discipline, up to and including discharge
  • Participates actively in quality assessment performance improvement teams and activities
  • Assists with clinical orientation and preceptor oversight
  • Assists Director of Patient Care Services and other supervisory personnel in the planning, implementation and evaluation of in-service and continuing education programs
  • Assists with assessment of staff's in service needs and participates in in-service programs
  • Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range
  • Assists in Administrative Call rotation and clinical call as needed
  • Checks and responds timely to emails and voice mail
  • Performs other duties as assigned

What we look for in a qualified candidate:

  • Registered nurse with current licensure to practice nursing in California
  • Possess and maintain CPR certification
  • Previous experience in home care setting with two years management or supervisory experience, preferred
  • Proven ability to work within an interdisciplinary setting
  • Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order
  • Prior use of Electronic Medical Records (EMR) preferred
  • Covid 19 Vaccination - Per a mandate issued by Los Angeles county, all healthcare workers must be fully vaccinated by 9/30/2021 to provide services to clients residing in the county.
