of our staff.Job SummaryProvide appropriate support with
information and guidance about pediatric medical and support
services. Serves as a liaison between internal and external
customers, including patients/clients, other FHCSD service staff
and other service providers, for the purpose of improving
patient/client quality of care and flow.Job Roles
- Facilitate care transitions between providers, partners,
referral sources and specialty care providers.Follow-up on patient
referrals for services and engagement in referral services within
established timeframes.
- Facilitate enrollment of patients in pediatric services to
include appointment scheduling, recalls and follow-up on missed
appointments. Specifically, scheduling circumcision referrals and
providing alternative community referrals to patients as
needed.
- Engage with patients and families in communicating the
importance of medical well-child exam visits and support services
visits.
- Complete all required documentation accurately and thoroughly,
in a timely manner, in accordance with department standards and
complies with patients' insurance requirements.
- Track, monitor, and actively manage pediatric patient registry
to ensure completion of well child exams, retention of patients,
and ensuring a high level of utilization is maintained.
- Maintain and communicate accurate information in CMIS to ensure
all activities are documented and conducted in compliance with
applicable regulatory requirements and accreditation
standards.
- Serves as a liaison between internal and external customers,
including patients/clients, other FHCSD service staff and other
service providers, for the purpose of improving patient/client
quality of care and flow.
- Works in collaboration with all members of the healthcare and
departmental program on retention, appointment completion, and
identifying barriers to care (e.g., quality improvement projects,
etc.).
- Engages in patient/client interactions via telephone and in
person to identify and meet patient/client needs, including
facilitation of assisted and unassisted referrals, resources, and
follow-up, depending on presenting needs.
- Performs general clerical tasks, as assigned, which may include
ensuring completion of paperwork by medical providers for patient
forms, data entry, scanning/photocopying, monthly scheduling
reports, and similar tasks.
- Uses discretion during all interactions to answer both routine
and complicated questions, forwards issues to appropriate person.
Handles confidential and routine information.
- Performs other duties as
assigned.Education/Certifications/Licenses/Registrations
- High School diploma (or GED equivalent) required.
- Traveling between sites and other locations is required as an
essential function of the job. Must have a car, a valid California
driver's license, and proof of minimum levels of car insurance as
required under California law, although limits of $100,000 are
recommended. An acceptable driving record is also required.
California law requires all drivers to obtain a valid California
driver's license within ten days of establishing residency. Mileage
and other reimbursement governed by policy.
- Specific Certifications may be required or preferred depending
on assignment (i.e.: CA HIV Test Counselor, limited CPT
certification for waived testing, etc.)Experience/Specialized
skills (including Language)
- 1 year of work experience providing services to medically
underserved or relevant community health populations required.
- Bilingual in Spanish/English required. Trilingual preferred:
English, Spanish and Arabic/Farsi/Dari/Pashto.
- Exceptional communication and problem-solving skills.

- Demonstrated ability to be culturally sensitive and respect
diversity.

- Demonstrated ability to exercise discretion and maintain
confidential information.
- Basic computer literacy ability to comply with department needs
and expectations (i.e., electronic medical record documentation,
obtaining background information and reports on patients, following
up on appointments, etc.).
- Basic supportive communication skills, such as reflecting,
active listening, and paraphrasing.
- Knowledge of health and social service systems in San Diego

fringe benefits.$22.00 - $26.17If you are hired at Family Health
Centers of San Diego, your final base salary compensation will be
determined based on factors such as geographic location,
jurisdictional requirements, skills, education, and/or experience.
In addition to these factors - we believe in the importance of pay
equity and consider internal equity of our current team members as
a part of any final offer. Please keep in mind that the range
mentioned above is what we reasonably expect to pay for the role.
Hiring at the maximum of the range would not be typical in order to
allow for future and continued salary growth. We also offer a
