weekend shifts.
Key Responsibilities
Continuously monitor patient vitals, symptoms, and lab results to
identify care gaps
Create and manage personalized care plans aligned with treatment
goals
Conduct virtual follow-up appointments and document clinical
interactions thoroughly
Required Qualifications
Multi-state RN Compact State Licensure
Associate Degree in Nursing Science
5+ years of clinical experience in chronic care management
Skilled in nursing processes and technical fluency with multiple
platforms
Patient assessment competency
