Job Description:
A company is looking for a Clinical Care Management Coordinator
to work from home.
Key Responsibilities
Initiate outbound calls for care management campaigns and gather
member-specific health data
Complete Health Risk Assessments and assist members in accessing
appropriate medical providers and community-based services
Collaborate with RN Case Managers and ensure accurate documentation
of all activities
Required Qualifications
Clinical background required; licensure not mandatory
Proficient in Microsoft Office and able to type at least 35 WPM
Demonstrates a professional presence both over the phone and in
person
Consistent attendance and reliability are essential
Ability to manage tasks effectively under pressure
